Schedule an appointment

To schedule a well-child visit, call our office during business hours. If your child receives care from one of our pediatricians, you also can schedule an appointment online using MyChart.

To make an appointment for a sick child, call our office at any time, or use MyChart to schedule for a child who receives care at our clinic. New patients, please contact us by phone to schedule a first-time visit.

Bring these items to your appointment:

• Photo ID
• The insurance card covering your child
• Co-payment or payment in full if your child’s visit will not be covered by insurance. (Click here for more insurance and payment information.)
• Forms needed for school, sports or camp physicals
• Medications your child is currently taking, including vitamins and other over-the-counter products
