School, camp and sports physicals
Your child may need a physical exam to attend school or camp or to participate in sports activities. A physical exam helps to ensure your child can participate without endangering himself or others. These exams also can help your pediatrician identify potentially serious medical problems.
Physical exams often can be completed during your child's annual checkup. You may incur fees for a separate visit. Please bring forms to your appointment if needed for school, camp or sports programs. Forms for Charlotte-Mecklenburg Schools athletic programs are available here.
